Since 2008, Singapore has been re-organising its health care delivery system into six regional health systems (RHS), each serving a defined geographic area comprising a population of about 0.8-1.2 million people. Health care providers will be linked by a national electronic health record system, drawing from localised electronic medical records that are interoperable and allow essential information to flow seamlessly from one RHS to another.
Concurrently, there are now efforts and initiatives to integrate health and social care providers, especially around long-term care for older people and community mental health.
